Medical, Dental & Vision Coverage


The first of the month following a 30 day waiting period, you will be eligible to enroll in medical benefits through our provider, Anthem.


The first of the month following a 30 day waiting period, you will also be eligible to participate in our premier Dental plan through our provider, Guardian.

Unsurpassed coverage with unused coverage roll-over and all employee premium contributions withheld on a pre-tax basis.


Vision coverage through VSP is offered the first of the month following a 30 day waiting period.

As you would expect, employee premium contributions are withheld on a pre-tax basis.

Life & AD&D Insurance

All full-time benefit eligible employees are provided one time his/her annual salary plus six months average bonus plus $25,000 to a maximum of $225,000 at no cost in life and AD&D insurance.

Long and Short-Term Disability

All full-time benefit eligible employees are provided standard long and short term disability coverage at no cost.

Health Savings Account (HSA)

If you elect to participate in the HDHP medical plan, you can deposit money to Forum on a pre-tax basis to pay for qualified medical expenses.

401K Retirement Plan

The first of the quarter following a six-month waiting period, you will be eligible to participate in our 401(k) plan offered through John Hancock.

Plan highlights include the ability to contribute different deferral amounts for regular and bonus earnings, personal retirement planning advice with a licensed Financial Advisor, annual employer discretionary match based on company profitability, and an annual employer contribution of $100 on your anniversary.
